Mrs V has some mental health issues, which she does not accept, and requires help and encouragement to complete personal care, monitor medication, meal preparation, and to remain involved in the community. Her daughter was very worried about her mother’s deteriorating health and mobility because she knew that she was not eating well and felt that the practical side of her mother’s care plan was not that effective.
After completing the new self-assessment a support plan has been agreed with Mrs V and her daughter that involves a ‘split package’ of care so, for example, morning calls are provided by the independent sector, and she now has evening calls and practical support from her daughter who will be her personal assistant and have direct payments.
Mrs V is currently in the process of having a stair lift installed to enable her to remain in her flat, which she loves.
